Eligibility / Target Population

Members of Pays Plat First Nation who require assistance with medical transportation due to illness, injury or age or have no access to a vehicle

Description of Services:

Delivers medical transportation services and assistance in making medical appointment arrangements

Enforces the medical patient transportation policy
Supervises the medical van driver as he/she provides transportation to any medical appointments within the community
Provides transportation from/to Pays Plat to any medical appointments (doctor, dentist, optometrist etc) in Schreiber/Terrace Bay, Nipigon and Thunder Bay
Provides assistance with making arrangements for any medical appointments out of town (includes bus warrants) such as booking appointments, hotels, and completing applications for mileage compensation
