  • Delivers medical transportation services and assistance in making medical appointment arrangements
  • Provides transportation to medical appointments within the community
  • Provides transportation from/to Long Lake 58 First Nation Reserve to medical appointments (doctor, dentist, optometrist, specialist) in Longlac, Geraldton and Thunder Bay
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments and hotels

  • Fees: None
    Eligibility - Population(s) Served: Members residing on Long Lake 58 First Nation who require medical transportation assistance due to age or disability
    Application:
    • Requires written proof of medical appointment

    • Requires 2 week advance booking; exceptions made for emergencies

    • Advance cancellation recommended
