  • Provides assistance with completing prior approval applications for long distance medical appointments
  • Offers assistance with arrangements for accommodation and travel details for individuals attending medical appointments at Sioux Lookout Meno Ya Win Health Centre

    Eligibility - Population(s) Served: Members of Slate Falls Nation who require assistance with medical transportation due to illness, injury or age

    • Requires written proof of medical appointment

    • Requires 24 hour advance booking; exceptions made for emergencies
