  • Provides escorted transportation to medical appointments, shopping, and to participate in various social activities and programs
  • Participants must meet the eligibility requirements

    Eligibility - Population(s) Served:
    • Individuals 18 years of age and older with specific needs such as disability, illness or injury that require assistance

    • Seniors 65 years of age and older

    • Medical appointments are a priority
    Application:
    • Requires completion of application or must be registered with the program

    • Must make arrangements at least 48 hours in advance

    • Cancellation notice should be given at least 24 hours in advance if possible
