Application
  • Requires completion of application or must be registered with the program

  • Must make arrangements at least 48 hours in advance

  • Cancellation notice should be given at least 24 hours in advance if possible
Eligibility / Target Population
  • Individuals 18 years of age and older with specific needs such as disability, illness or injury that require assistance

  • Seniors 65 years of age and older

  • Medical appointments are a priority
