Description of Services:	Provides a monthly diabetes clinic which offers education and services for the prevention, management and treatment of diabetes Provides case management and screening services Offers basic foot care services Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and management of diabetes Communicates patients' concerns to family physician Assists individuals in finding appropriate services Provides referrals to other health care professionals

Eligibility - Population(s) Served:	Individuals diagnosed with diabetes or high-risk

Application:	Maintains list of diabetic patients; will contact individuals with date/time of next clinic
