Description of Services: Provides services and supports to assist individuals to achieve and maintain a quality life
  • Assists individual in developing a realistic plan to acquire the skills and supports to enable them to live independently
  • Determines areas of need and develops the support plan with a focus on each individual
  • Provides referrals to community resources and supports
  • Provides the supports in a community setting or within individual's home environment
  • Focuses on wellness and recovery
Supports can include assistance in the following areas:
  • Daily living skills, basic needs and life and social skills
  • Employment
  • Navigating the mental health system
  • Accessing medical and psychiatric services
  • Medication and symptom management
  • Crisis prevention, intervention and management

Fees: None
Eligibility - Population(s) Served:
  • Individuals 18 years of age and older diagnosed with a serious mental illness

  • Clients must voluntarily agree to services

Application:
  • Intake assessment required

  • Support can be brief or long-term
