Requires written proof of medical appointment
Requires 24 hour advance booking; exceptions made for emergencies
Advance cancellation recommended

Out-of-Town Medical Assistance

Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel to allow time to process Prior Approval package with Health Canada
Must have Confirmation of Appointment (COA-blue slip) stamped/signed and return to Health Centre after appointment

Description of Services:

Delivers medical transportation services and assistance with processing Non-Insured Health benefits claims

Provides transportation from/to Couchiching Reserve to any medical appointments (doctor, dentist, optometrist etc) in the town of Fort Frances
Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual for out-of-town medical appointments through the Non-Insured Health benefits program
Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
