Description of Services:	Provides case management services to adults living with a mental illness and a developmental disability Promotes recovery and independence while providing appropriate community based support Provides the following services: Assistance in accessing mental health and/or psychiatric services Help to integrate into the community, advocating with informal and formal community resources Referrals to community agencies, where appropriate Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program Housing rent supplement Assistance in obtaining and maintaining housing in the community

Eligibility - Population(s) Served:	Individuals 16 years of age and older who have a mental illness and a developmental disability

Application:	Can self-refer or be referred by a family member or agency worker Apply through the Northwest Access Point
