  • Must be of Aboriginal descent

  • Must be 55 years of age and older

  • Must be frail, vulnerable, and at risk

  • Must have one or more chronic illnesses or disabilities

  • Has limited access to culturally appropriate services
Application: Referrals can be made by anyone in community
