Eligibility / Target Population

Patricia Gardens residents and community members 60 years of age and over

Must meet one of the following criteria:

The client is in receipt of or eligible to receive other long term care community services
The client is isolated and requires social interaction or stimulation

Description of Services:

Provides a blood pressure clinic once per month

Offers health check-ups with a Nurse Practitioner twice per month

Provides advice on health issues and offers referrals to other health providers and services
