Description of Services: Provides off-site supervision and support to member communities in their implementation of the Community Wellness Worker (CWW) program
  • Promotes healthy lifestyles through culturally based programs and healing
  • Addresses and aims to reduce family violence
Program delivered at the community level consists of the following services:
  • Provides assistance for individuals to identify areas of concern and to develop action plans to address concerns
  • Liaises and networks with related health and social agencies and provides referrals
  • Provides crisis intervention education and workshops
  • Offers illness prevention and wellness planning, information and education
  • Provides community outreach education to promote cultural awareness
