Description of Services:

Provides services that promote physical and mental health for community members

Coordinates health care services through Community Health Nurse, Community Health Representative, Clerk and Diabetes worker as well as other professionals
Provides health awareness and counselling, education and prevention workshops
Coordinates clinics/workshops in areas that may include Immunization, Sexual Health, Smoking Cessation and Communicable Diseases
Administers a Safe Needle Exchange program and functions as the safe needle exchange and safe inhalation kit pick-up and drop-off site

Addresses individual health care needs by providing:

Intake services to determine health care needs of individual
Referrals to appropriate health care providers based on information in the intake assessment
Advocacy and support on behalf of individual with other health professionals
Coordination of one-on-one pre-natal care with the community health nurse
Home visits to the sick, elderly, new mothers and persons with health care needs
Assistance with coordinating local and long distance travel arrangements for medical appointments
