  • Provides a comprehensive geriatric assessment via telemedicine (using videoconferencing and other electronic equipment) with a focus on identifying resources and strengths of the client
  • Community services are identified and/or initiated to keep clients in their home and community and a coordinated care plan is delivered based on the assessment needs of the client and their families

  • Eligibility - Population(s) Served: Seniors 65 years and older or physiologically and/or cognitively equivalent who have:

    • deteriorating health or functional status

    • safety concerns

    • frequent or increased use of health services

    • altered quality of life


    Residency Requirements: For seniors living throughout Northwestern Ontario (Clients living in the city of Thunder Bay can access Geriatric services at St Joseph's Hospital)
    Application: Physician/Nurse Practitioner referral is required, though clients without family doctors can access referrals through walk in clinics
