Description of Services:

A specialized outpatient program to support individuals with wounds that are slow to heal

Supports the physical, mental, emotional, environmental, nutritional and financial needs of the individual
Provides consultation with other healthcare providers as needed
Provides Telemedicine appointments as appropriate
Offers a multidisciplinary team consisting of Nurse specialized in wound, ostomy and continence, Physiotherapist, Occupational Therapist, Dietitian, Social Worker, Chiropodist, and Physician
