Description of Services:

Ontario Health atHome Medical Assistance in Dying (MAiD) Coordinator provides:

Assistance to support health care providers, patients and family caregivers to access resources and to help connect to clinicians who are willing to provide Medical Assistance in Dying services
Support for patients to understand the process of MAiD
Help to make a request for MAiD and to connect to local clinicians who can help patients access palliative care services or medical assistance in dying
