Description of Services:

Provides services for community seniors to enable them to remain healthy and live independently as long as possible

Offers home visits by a Registered Nurse or Nurse Practitioner following an initial home assessment
Monitors medical concerns, illness prevention and offers health promotion
Provides nutrition assessments to enable advice about healthy eating and signs of aging such as osteoporosis
Assists in obtaining the Lifeline program, an emergency help line reachable at the push of a button
Provides referrals to therapists and geriatric experts
