  • Requires a referral from Physician

  • Comprehensive assessment with patient and family members will be booked; appointment lasts approximately two hours

  • Package will be provided for patient to complete prior to initial appointment

Individuals with Alzheimer Disease or dementia, their families and professional caregivers

Description of Services:
Provides a clinic for early diagnosing of cognitive impairments
  • Partners with the Sunset Country Family Health Team to offer the memory clinic
  • Provides early diagnosis and treatment for problems associated with memory loss
  • Team includes Physicians, Nurse Practitioner, Registered Nurse, Registered Practical Nurse, Social Worker, Dietitian, Pharmacist and Client Services Coordinator with Alzheimer Society
