Description of Services:
  • Provides support to patients after being discharged from hospital
  • Sends letters as well to patients having surgery to contact clinic for follow-up assistance post-surgery
    • After hospital discharge services include:
    • A phone call from RN to ensure there are no problems
    • Assistance with symptom management
    • Help with making arrangements for home care if required
    • Booking a follow-up appointment with Physician
    • Home visits by RN if patient is struggling in any area
