Eligibility / Target Population
Individuals 18 years of age and older

  • Must have a valid Ontario Health Insurance Plan (OHIP) card

  • Must have health care needs that cannot be met with any combination of caregiving, care in your home or care in your community and

  • Must have health care needs that can be met in a long term care home
Fees
Program - Some programs may require additional fees; Service - Accommodation; Fee assistance available for Basic accommodations; assistance is provided by Ontario Health

Description of Services:
26-bed long term care facility offering a variety of programs, services and amenities
Coordinates and delivers the following programs to residents:
  • Community special events
  • Facility tours
  • Guest speakers
  • Immunizations for influenza and pneumonia
  • Memorial services
  • Pet visitation and therapy
  • Volunteer appreciation events
  • Voting day polling station
Provides residents with the following services:
  • 24-hour nursing care and supervision
  • 24-hour on call physician
  • Foot care
  • Hairdressing and barber services
  • Laundry services
  • Meals and snacks
  • Medical devices and equipment
  • Medication administration
  • Palliative care (end-of-life care)
  • Physician visit
  • Postal service
  • Social work services and family services coordinator
  • Spiritual programs and pastoral care
  • Supplies and equipment for personal hygiene and grooming
  • Various recreational programs and social events
Provides the following amenities:
  • Air-conditioning
  • Common dining room
  • Common television room
  • Private and semi-private rooms with 2-piece washroom (toilet and sink)
  • Shared shower and bathing room
  • Secure building units
  • Secure units
