Description of Services:

Provides culturally appropriate and holistic long term support services and programs including the following:

Access to community supports
Assistance to coordinate medical appointments
Congregate dining
Cultural programs
Home visits
Physical fitness
Referrals and advocacy
Social activities

Transportation Program

Provides transportation on an as needed basis to all events at the Centre, the hospital or clinic and for errands around town including grocery shopping
Arranges transportation for out of town specialist appointments when no other arrangements are available
