Eligibility / Target Population
- Must have a valid Ontario Health Card
- Must have health care needs that cannot be met with any combination of caregiving, care in your home or care in your community
- Must have health care needs that can be met in a long term care home
Fees
Service - Accommodation; Some services may require additional fees ; Fee assistance available for Basic accommodations ; assistance is provided by Ministry of Health and Long-Term Care (MOHLTC).
