Description of Services:
Offers blood pressure monitoring and education
Educates individuals about high blood pressure (hypertension) to understand the risks and strategies for lowering blood pressure
Provides education and information for:
Self-monitoring of blood pressure and management through diet, exercise, stress reduction and smoking cessation
Cholesterol management
Congestive heart failure (CHF)
Stroke prevention
Cardiac Education Program
Offers post event cardiac education
Includes individual assessment, education and support
Develops treatment/education goals for each person and reviews recent event, heart health, blood pressure management and medication use
Offers nutrition and healthy lifestyle education with Registered Dietitian
Conducts weekly group educational sessions
