Description of Services:

Offers a mobile Indigenous Interdisciplinary Primary Care Team, providing services to the Sioux Lookout First Nation Health Authority communities as well as the Municipality of Sioux Lookout
Teams will travel to communities on a regular basis to provide collaborative care in the following areas:

Community Health Workers
Nursing (Registered Nurses, Registered Practical Nurses, Nurse Practitioners)
Nutritional Consultations and Food Care
Occupational Therapy
Pharmacy Consultation
Physiotherapy and Kinesiology
Pharmacist
Registered Dietician
Speech Therapy
Telemedicine Services
Traditional Healing
