Referral by local Physician and a Registered Dietitian
Eligibility / Target Population
Patients 18 years of age and older of the Family Health Team who meet three or more of the criteria for Metabolic Syndrome including the following:

  • High blood sugars

  • Abnormal cholesterol

  • High blood pressure

  • High triglycerides

  • High abdominal body fat

Description of Services:
Lifestyle intervention program
  • Offers a six-month program offered by a team of healthcare professionals including a Dietitian and Physiotherapist
  • Offered in a group setting of 15 participants
