  • Creates an individualized care plan based on a person's needs and goals

    • cultural and religous activities
    • education
    • employment
    • housing
    • life skills
    • medical needs (dental, vision, mental health)
    • shopping

    Eligibility - Population(s) Served: Adults 18 years of age and older with a developmental disability as well as significant medical or behavioural support needs
